HOW TO GET STARTED
You can schedule an appointment without seeing your doctor per NYS Direct Access Law. This allows us to see you for a total of 10 visits or 30 days before obtaining a prescription from a doctor. You may also have a referral from a physician. This is required for patients with No Fault, Workman’s Compensation and Medicare Insurance–
Schedule an Initial Appointment
At SportsFocus Physical Therapy, the process begins with an introductory interview and active listening to our patients as you tell us about your physical symptoms, functional complaints and goals. This is promptly followed by a thorough physical examination and evaluation to determine the possible root causes of these complaints. Based on our interview and examination results, we then explain in detail to our patients and answer your questions in language that can easily be understood, regarding the nature, significance and prognosis of our clinical findings, as well as the rationale behind physical therapy intervention.
This is just the introduction of offering active listening, comprehensive examination and meaningful education to our patients, as these elements are a common thread that connects each visit with us throughout your episode of care with us.
And the word “care” is an important one to the Staff at SportsFocus. We feel that this practice is indeed one where caring and effective results meet. We don’t simply regard our patients as a collection of complaints to be remedied, but as a “real live person” to share physical therapy care with, to aid in your healing and recovery from the physical issues that may interfere with living your life in the manner that you wish.
Our physical therapy care centers around customized, evidence based therapeutic exercise interventions. Some of this therapeutic exercise will be performed at home, school or your health club, and others are designed to be performed specifically while at our facilities. This type of active care is designed specifically for each patient based upon our clinical findings, physical capacities and goals, and intended to be as effective and efficient as possible.
Simply put, physical therapy at SportsFocus is centered upon our wish and intention to be of benefit to every patient who is kind enough to choose our practice out of the many other choices, for their physical therapy needs. We aspire to be of benefit by not only being professionally skillful, effective and efficient, but also by being caring, supportive and compassionate as well.
Your First Appointment
For your first appointment, you will need to arrive to the office 30 min before you are scheduled so that we can have you fill out the required paperwork.
You will need to bring with you:
- Your prescription or referral from your doctor if you have one
- Your insurance card
- A list of your current medications
- Copies of any diagnostic testing (MRI, CT, XRay) or operative reports you may have
Please wear comfortable clothing to make it easy to move. If you have a leg issue, wear something that allows us to view the knee and a sleeveless shirt if you are experiencing a shoulder/elbow issue.
If you wish to fill out the paperwork in advance, you can then arrive 10 minutes before your appointment time.

- Medical History (all patients must fill this out)
For the Demographic/Insurance Information, choose the insurance type that you will be using:
- If you were involved in a car accident and using No Fault insurance
- If you were involved in a work related injury and using Workman’s Compensation insurance
- For all other insurances or if not using any insurance

We are considered out-of-network providers for most Aetna, Cigna and United Health Care plans. You will have to contact your insurance company and find out your out-of-network benefits and any deductible you might have.
If you do not have insurance or have exhausted your insurance benefits, please contact the office and we can let you know the cost of the initial and follow-up visits.
Copays/deductibles are due at the time of each visit.

Cancellation/No-Show Policy
We request that you call as soon as possible if you are unable to make a scheduled appointment. However we understand that there are many legitimate and unforeseeable reasons for being unable to attend a scheduled visit. We will allow a total of 3 cancellations or no-shows during your physical therapy care before charging $25 for each future cancellation or no-show.
